Motorists pay more for their car insurance in the cities than they do in other areas, the British Insurance Brokers Association (Biba) has said.
This is due to higher volumes of traffic and higher crime and accident rates.
There are also now more than two million uninsured drivers on the UK's roads and many of them are in the large towns and cities.
Six out of ten of the worst offending places were in the West Midlands, according to the Motor Insurance Bureau.
Graeme Trudgill, Biba technical and corporate affairs executive, believes this is due to people on a low income being unable to afford the high cost of living in places like Birmingham.
"One of the main reasons for putting people off buying insurance in the middle of Birmingham or middle of London is that it's extremely expensive compared to if you're living in a nice village in Wales," he said.
Biba has also warned people not to try to evade car insurance because of the expense.
According to research by Direct Line, 200 motorists a year are killed in accidents involving uninsured drivers.
